Primary evidence of earnings includes pay stubs and earnings statements prepared and signed by the employer.
Primary evidence of earnings can be provided:
- On company letterhead
- On the Employer-Prepared Earnings Statement form found in the Information Center Forms page under “Payments.”
- On records from a third-party source such as The Work Number at http://www.theworknumber.com.
If the EN submits pay stubs, the stubs must show the beginning and ending dates for the pay period, the pay date, gross earnings, and withholdings (FICA, Medicare, etc.).
